I don't have a set plan as such but I think it's important to be in a talkative state (I'm naturally chatty so I don't face this issue like some). Make conversation with random people throughout your day. An idea could be to talk to a person who looks up to you or younger sibling to establish a commanding tone... not necessary, but it's important to take a stance of authority to some extent on dates.
This is as much of a pre-date warm up as you can get. It preps you for being in a chatty vibe, gets you in the state to think on your feet.
I used to get stage fright so I'd thought lately of incorporating a quick meditation or breathing exercise beforehand, maybe a glass of wine or whisky after. Or I might chill by going to the gym beforehand -- followed by a shower.
Honestly, organisation was never my forte; I know most of my past dates I'd have been rushing from somewhere. Somehow, I'd manage to just segue into date mode like that. I ought to cut that out.
I dress pretty smartly -- so my dates aren't a huge exception, though I try not to look like I've tried too hard. I'll usually wear a jacket of some sort or a pullover.
Often, I'll incorporate something that I already know about the girl into my outfit. If she's a hipster I'll wear something ironic -- much of my wardrobe are vintage/thrift finds so I've got that covered anyway.
I once wore an army surplus jacket on a date* where the girl happened to do WW2 reenactment so that went down pretty well.
