Agree and amplify. Depends on the scenario.
What you did sounds like way too much effort for one chick though. Editing a picture of her is borderline creepy.

I could be wrong, but I think he meant he attached a real picture of her that already existed on her Facebook. Doesn't seem like a big deal... first thing that pops into my head is "No, of course not." with a sarcastic grin. Seems like very casual/playful comment though, not some big shit test. I wouldn't worry much about it.

I'd say it depends on the edit. Photoshopping is freaky. MS paint + cut tool, making it look like a 5-year-old did it, can be kind of funny.
As for "making fun of me?" response, why not just roll with what feels comfortable? Or just ignore the question.
