I've spent all morning reading these resources from Venture Hacks:
http://venturehacks.com/friends
The first resource, Pitching Hacks, is the closest thing I've ever found to a "blueprint" for raising a round of venture capital.
The second resource is a collection of all of the Venture Hacks blog posts that cover the same topic, albeit in a wider range of contexts. It's less organized but still highly valuable.
The first resource is an actual cap table and instructions on how to fill out the cap table.
I highly recommend reading up on these resources if you are considering raising capital.
Another great resource is Venture Deals: Be Smarter than your Lawyer and Venture Capitalist
