I have not tried it myself but from what I have heard from other travellers - Panama, Angeles City in the Philippines.

This probably depends a lot on what particular health services you need, but in my (limited) experience, Chiang Mai provided great service at a reasonable price, while Penang in Malaysia (considered a medical tourism destination) was comparatively unpleasant, and surprisingly expensive for the simple procedure I required. I've only been to the doctor once in each place though, so I wouldn't be surprised if it turned out differently for someone else. I can imagine that someone might have a poor experience in CM if they ended up choosing the wrong provider, while this is unlikely to happen in Penang if you go to a large hospital. Penang will most likely be more expensive, but (much?) less expensive than the US. I avoided the hospital in Penang, and went around trying to find a small clinic thinking it would be cheaper, but it doesn't seem like it was.
Taiwan is also considered to have cheap and good healthcare, and my (again, only once) experience matches this reputation, so it could be an option.

Taiwan has excellent healthcare to an European standard for relatively low prices, but language issues might come up more than in places like Malaysia.

Its been mentioned in this thread already but Malaysia and Thailand have very awesome healthcare and are commonplace for medical tourism.
Went and visited the doctor in Kuala Lumpur, I was totally shocked by the level of service. I walked in, everything was clean and modern and they spoke perfect English. It looked private run, first thing I thought was damn, this is going to break my wallet. I expected to pay at least £100. Walked out medicine in hand and only cost £16. I knew what was wrong with me and told them what drugs I'd like, she was like "yep thats not a problem". Really happy.
