I've experienced this type of thing mostly due to inexperience. Previously had a girl not willing to do oral because of no experience and frankly being scared of it. For some it's like going from regular sex to anal - it freaks them out.
As for the girl unwilling to do anything more than missionary, you should have just done what you wanted with her. A girl like that likes exercising control in the bedroom and you shouldn't allow that as an alpha. Every girl wants to be thrown up against a wall, so why not.
For the oral situation or the hand job, I think sometimes you literally have to just *show them* how to do it. Why waste 10 minutes of a hand/blow job with them doing it terribly. Show them how and enjoy the benefits. Just don't be a dick about it.
