Call them up and ask to have it added. You may also want to add an international data package, which is separate from the voice services, otherwise you will rack up huge charges if you use data.
Other providers should have similar packages. The whole CDMA (e.g. Verizon)/GSM (e.g. AT&T) thing is not really an issue in Canada, because Canada has both kinds of networks, unlike many other places, which have just GSM.
So I checked out the ATT one: $0.59/min. roaming in Canada
I think T-mobile has the same.

Yeah. That's their given rate without a plan. Call them up and ask about their international Canada rate. Let them know your situation. They don't always advertise those plans.
