Cities with the most English-Speaking Expats

I am a health care professional and would like to permanently leave the USA. I don't want to be specific about the field for privacy reasons.
However, my chances of practicing would be better if I can find a city with the most English speaking Expats. Of course, London might be one but the chicks are... Anglodefeminized.
This might also be relevant for anyone who wants to provide a service that is not done over the Internet.
So I'm kind of wondering if anyone knows much about it.
I guess the femininity of the women is a factor too.
Some ideas that come to mind are Madrid, Moscow.. I don't know...

Hong Kong and Singapore both have large english speaking population. The girls are just average in looks compared to surrounding countries. Both cities are great hubs and easy to use as a place to visit other parts of asia. Cost of living is relatively high compared to other places in Asia but you can also expect to make more money if you are working in the local economy.

In Europe it would be cities in Southern Spain like Marbella, but with free healthcare, I am not sure it would be worth your while. Other places are Malta and Cyprus.
Amsterdam is also good for English speakers, as is Stockholm.

In Europe, I'd say Berlin tops it.
The number of foreigners is pretty stagering, and not just tourists or turks.
Poland and the czech republic being nearby, that gives you an easy escape for the weekends too.

According to Wikipedia, at least 440k americans live in Mexico City.
And with health tourism on the rise, I see you have quite a niche there.
Live in Mexico, advertise in the US. Sell them the "American health professionals, mexican prices" idea, and charge them something in the middle.

In terms of high nett worth expats with private health insurance Geneva would be another one BUT the quality of women and ratios etc are very poor according to other threads + its expensive to live there. There must be some stats on expats somewhere.
