The key is to identify your triggers and move away from them, consciously. Easier said then done, but that's why this thread is 139 pages.

In the meantime, I highly recommend some kind of porn blocker on your computer. My favorite is Self-Control for Mac, because it allows you to block porn sites and "other" sites that might trigger you, like reddit or twitter.
