I used to use Hushmail, but suspect it has sold out.
Nothing electronic is 100% secure.
Using Cypherpunk anonymous remailer is close to as good as privacy gets, unless you want to use couriers and live in a cave.

I used to use a disposable email called slopsbox, but it's no longer around. From what I've heard though YOPmail is basically its replacement. You can also mask the actual email address with it too.

Buy non-US controlled domain using non-US registrar. I chose .pe as first two letters of my surname so I can have
[email protected] email.
Buy cheap server at ovh.com in France and hide it under Eastern European IP (that you can buy from them as well). You need a dedicated server, not a VPS, so you don't have any "neighbours" on the physical box.
I bough one from soyoustart.com and added Belgium and Netherlands IPs.
Install this

Enjoy your privacy. Email, chat, dropbox-like file storage, calendar, VPN, all included.
Total cost: $50 per year for the domain name and $30 to $60 per month for a dedicated server.

If you don't own the hardware, you aren't secure. I wouldn't trust an off shore server company just as much as I don't trust Google to not let the NSA know about what genre of porn I enjoy the most.
Buy your own domain, run the server at your house, and pay for the bandwidth. Use PGP to encrypt your email.

There have been news reports saying that NSA can/could hack SSL. So, even offshore accounts are not secure.

I'm surprised that no one has mentioned Email services based in Germany!
GMX has long been a security conscious favorite. "Posteo" fought off German police requests; it offers anonymity and does not keep customer IDs.

Quite ironic that Germany of all countries is anal with their security. Less than 40 years ago the Stasi was spying on everyone and their mom in Deutschland!
Anyway I just came across tutanota.de for email services. Seems legit so far

Hate to break it to you guys but if YOU ARE IN US and you send emails OUT of US, I believe the gov can snoop on you. There was even a story I think of Tormail or whatever that was called that was associated with Tor where all the members info was gathered. Just accept the fact it's 1984
