I can throw in a couple of cents into this discussion, because it's an area that I often encounter.
I'm naturally (even when I'm not gaming) sort of an insulting, taunting dude--especially with girls. I've just come to accept that I don't have a lot of tact, and I'm not really making any effort to develop any. I get "called out" by chicks all the time, which often means they say something like, "you're mean. I'm not talking to you anymore," and try to leave. The great part is that they often say it with a provocative smirk on their face. It's my moment to capture or lose.
I usually turn things around by exaggeratively admitting that "I was wrong," and then immediately doing something else offensive or by overtly escalating to physical affection to "make up for it." It's a natural part of my personality, and probably wouldn't work with a lot of dudes that have a more serious or mysterious persona. Shit has to be congruent.
Calling your girl by a dog's name is definitely a thing you have to prepared to back away from. If a girl takes offense, it's actually okay to sometimes admit you're wrong. You just have to find a way to do it in a way that preserves your unique manly frame. That varies from dude to dude, so choose your own adventure.
