As far as the issue of running into online girls around town and vice versa it happened to me all the time when I lived in a town of ~100,000 people. It did result in one notch. A girl came up to me at a party I was hosting after it was winding down. She kissed me and then said she'd seen my profile online and had a crush on me. Then we went back to my room and banged. Go figure.
In general my strategy was to ignore it and approach the girl as usual unless there was some kind of deal breaker (like having a kid) in which case I'd just move on. More awkward was running into girls I'd gone out with once or twice while they were on dates with other dudes but that's just life in a smaller town.
