It happened with my ex of 6 years. I was trying very specifically NOT to get her preggo and she was on birth control (although she didn't have enough sense to take it properly as she would skip days and then take 2 pills the next day). I pulled out every time we had sex. So she either got preggo from pre cum OR she got got preggo after I had already blown it once and didn't go to the bathroom to clean out my urethra (which was rare since we usually just had sex once and that was it).
