Dr Jeffry Life, a Las Vegas-based age management doctor, is 74. He has the body of a man half his age. In fact he has a level of muscular definition that many men never achieve.

Genial, soft spoken, bald and lean, Life, who practised family medicine for much of his career, is a poster child for the longevity business.

He was in his late 50s when he realised that as a paunchy middle-aged man, he could drop dead at any time.

"I had really got out of shape, got a lot of body fat and was heading down a disastrous course of diabetes and heart disease," he says.

Inspired by a muscle magazine that someone left in his examination room, Life decided to get in shape. He started an intensive regime of working out at the gym and within a year transformed his body.

In 1998 he entered a competition, the Body for Life contest, for people who have made dramatic changes to their physique. To his surprise he won first prize in the section for 55-year-olds and older.

"It changed my life and I felt great," he says.

But as he got older he says he noticed that he was losing ground and finding it more difficult to maintain a lean body. While in his 60s, he visited Las Vegas for a medical conference and was introduced to Cenegenics, an age management company that aspired to make its patients "look and feel years younger". He went on to become a senior partner.

Life believes that the right kind of exercise and nutrition are important - but correcting hormone deficiencies are the key to his success.

"I got my blood checked and I found that I was profoundly deficient in testosterone," he says.

He now has weekly injections of the hormone.

"The problem is that when a man's testosterone gets low they lose their incentive to go to the gym.

"Even if they want to, they dread going to the gym and exercising and it's a losing battle. Especially abdominal fat around their belly. They spiral out of control.

"A lot of men come to me who are suffering from male-menopause, known as andropause, which creeps into their lives," he adds.

Andropause, says Life, is characterised by a decline in a man's sexual function, cognitive ability, an inability to get rid of body fat and fatigue.

He adds: "When I get their testosterone up to a healthy level, it changes everything. They get re-energised, they start seeing body fat disappear and muscle growing."

Different laboratories vary in what they consider to be a normal testosterone level. According to the Facey Medical Group, a health provider California, the normal reference range is 250 to 1100ng/dl. The Mayo Clinic says testosterone levels drop, within this range, by about 1% a year after the age of 30.

Andropause is not universally accepted by the medical profession as a definable condition for middle-aged and older men, although there has been a huge increase in the number of prescriptions for testosterone in recent years. As well as injections, gels are available and are widely advertised. Last year the British Medical Journal published a study which concluded that "many men in the UK are receiving unnecessary testosterone replacement".

It is an area of medicine that is subject to much debate and often focuses on the question of what is "normal" ageing.

Life argues that the medical profession is too conservative.

"Many doctors get the numbers and then they will tell their patient, 'Joe, your levels are normal', when in fact it is within the reference range, but it's at the bottom of the range. They don't tell you you're a D-student," he says.

Sex hormones - oestrogen in woman and testosterone in men - fulfil many roles in the body. Studies have shown that they are involved in age-related changes, such as the development of dementia.

Testosterone is fantastic," says Dr Christian Pike, a neurobiologist at the University of Southern California, who specialises in the brain and Alzheimer's disease.

"It increases aspects of cognition, it protects the brain from dying, it reduces Alzheimer's disease, I mean, it's wonderful," he says.

But Dr Pike has reservations about the recent trend towards an increased use of sex hormones to reverse the effects of ageing. He says further long-term research is needed to fully understand the way testosterone affects the body.

"It makes me a tad nervous," he says.

"We know that prostate tumours respond to testosterone with incredible growth."

Pike says there is promising research focusing on what can be done to turn on the brain's own testosterone-making system. He says relatively higher levels of testosterone are associated with greater longevity and that people who live longer have better levels of the hormone.

The challenge, he says, is balancing the benefits and the risks.

"So many of the factors that we are looking at are double-edged swords."

Even if testosterone and other forms of HRT do not actually extend lifespan, they would still be worthwhile in my opinion for increasing total quality of life. For example, I'd rather live to 75 or 80 while maintaining my vigor, muscle mass and sex drive than to live to 100 but be rendered a weak, sexless shell of myself for the last 30 years of my life. I definitely plan on availing myself of the full range of HRT options when I get into my 50s.

I was under the impression high testosterone levels led to more risk taking, and would be more likely to decrease lifespan?

OK guys, I do not think that testosterone will make you live longer, but it will allow you to maintain a GREAT quality of life and help you avoid alot of the ills that come with ageing. Remember, we don't die of old age, we die of the complications and degenerative effects of ageing. After years of taking testosterone (I used to be a pro athlete), I can attest to the amazing benefits of the hormone. I am currently on a trt regime that is a product of years of experience with the hormone, scientific studies and guidance from hormone specialists (a doc who specialised in doping) Here is the trt protocol that I recommend.

25mg of testosterone enanthate/cypionate to administered on M/W/F by subcutaneous injection. Use the small insulin needles for this and inject into the fat around the stomach.

The smaller and more frequent injections increase the level of free testosterone in your system and also almost totally eliminates any conversion into aromatase (which increases estrogen).

75mg is more than enough for 95% of guys and you want your test in the 900 ng/dl range. Any more than a 1000 ng/dl and you will need things like AI's (aromatase inhibitors) and all sorts of ancillaries. Remember that this is trt and NOT a steroid cycle.

As you ar administering exogenous testosterone it signals your HPTA (hypothalamic-pituitary-testicular axis) to stop producing testosterone. now with this protocol you will NOT be totally shut down, another reason why we use the more frequent smaller injections instead of a once a week shot.

To keep a % of your own testosterone running you will need to add the following supplements to your trt protocol.

D aspartic acid 3g per day (3 weeks on/1 week off) this amino acid signals your hpta to produce testosterone and increases steroidogenesis (production of steroid hormones like testosterone) in the testicles (where most of your bodies test is produced).

6 raw eggs per day - provides your body with all of that healthy cholesterol and allows the D aspartic acid to do it's work. The eggs and the DAA are the most important part of the trt protocol apart from the test.

Optizinc - 25mg per day

DIM - 200 mg per day (this is for reducing E2, the nasty cancer causing estrogen and for prostate protection)

1 teaspoon of pumpkin seed oil (also for prostate protection)

1 tablespoon of high strength fish oil (for general health and a health cholesterol profile).

As long as your balls are nice and full, it means that your body is still producing a fair bit of it's own testosterone. You'll want to have a blood test about a month into this protocol to check that your test levels are where you want them to be. you'll also want to get your E2 checked (estradiol) and make sure it's between 30 - 50 pmol/L.

As long as your blood tests are ok you'll only require a bi yearly blood test. Now i'm not a Doc so this advice is for 'entertainment and hypothetical purposes only'. But I will say one thing, it is a cutting edge trt protocol and you will not find a better protocol than the one outlined above. If you're under 30 of your test levels are above 500ng/dl then you don't really need trt.

Also check your local laws as in some localities the possession of hormones is illegal. The bro's in the Uk and Canada are ok. I hope that some bro's will get some benefit from this post.

He's basically taking steroids. It's generally not recommended for younger guys to do steroids or testosterone. You're body is still growing and working normally. 35+ would be a good time to try things like this.

I'll admit I've tried Androstendione back when Mark McGwire was doing it. It was over the counter. You could buy the stuff on eBay. You take 1-2 100mg capsules an hour before lifting and you're all charged up and feeling great. My appetite sky rocketed. I lost lots of weight the first two months. It also makes you shit like 5 times a day. Obviously because you're eating like a horse.

I'm not against steroids or Testosterone use. Just do your homework and use it in small doses and learn how cycling works.

It's amazing how much better you feel. Stronger, happier, clear headed. You'll also sleep like a rock.

Pretty ignorant comment, but hey...

I did say in my post that trt is not recommended for younger guys and I did say that this protocol IS NOT A STEROID CYCLE. For the younger cats you could try applying the rest of the protocol without the testosterone injections.

I don't see the need to go this route if you are under 40.

Your T-levels decline when you hit 30 but they don't fall like the NY stock exchange in 2008. I think a good diet and exercise plan can make your decline rather slow.

I routinely here guys on the forum who still report having that "I'd fuck anything, anytime, anyplace," feeling well into their 40s. Then I hear guys who are 35 looking for T-boosting tips since they can't figure out why they are not as horny as they were when they were 27. Almost always the latter is not living a healthy lifestyle (one guy reported smoking and drinking a lot).

Well, there you go.
