Report her to the police for criminal assault and battery, especially if you have a witness.
Even if you decide not to press charges, just having her arrested and brought in for questioning will be a good scare/hassle for her, and she will subsequently have the beginning of an arrest record.

Why didn't you act in the moment? Time passing really weakens your case, both in the legal sense (calling the cops) or in the social-sympathy case (some kind of payback).
